Dynamics | How do we install the SMS for Dynamics solution to an on-prem CRM?

This App is developed from an online (cloud) version of Dynamics 365 and is compatible with an online instance of Dynamics 365. SMS for Dynamics will not work on Dynamics 365 on-premise.
